With the public relying on their health care providers now more than ever before, health and social care professionals’ easy access to a patient’s entire health and care history has never been more important. However, with patient records being held separately by numerous health and social care providers as standard, obtaining a holistic view of a citizen’s history has never been straightforward. Any delays experienced in accessing critical information can result in slower clinical decisions and inevitably, delaying patients receiving their treatments and care plans. Clearly, an efficient and streamlined solution is needed.
Consequently, there came a directive from the UK Government to introduce Shared Care Records. Kent and Medway’s Shared Care Record is a key deliverable of the Kent and Medway Long Term Plan. Cantium – working on behalf of the commissioning partners (KCC, Medway Council and Kent and Medway CCG) – has delivered a centralised Kent and Medway Care Record (KMCR) that collates numerous data streams from the county’s health and social care providers, into one comprehensive electronic database.
Cantium’s KMCR initiative provides a joined-up view of the history and plans of a resident’s health and care by collating individual data streams from separate health and social care organisations into one comprehensive feed. The KMCR contains automated, regular data feeds from health and care service providers including GPs and social care departments.
The development of the KMCR as an integrated health and social care record required input from clinical and health care professionals and providers throughout Kent and Medway. In conjunction with Cantium’s public sector knowledge, including experience of parterning with the NHS, this ensured this digital database was an informed solution to what is a complicated problem.
Currently operating in the Kent and Medway region, the KMCR is bridging the gap between different providers and building an information channel that’s easily accessible for all health professionals.

In response to the pandemic, the KMCR project has also included the mobilisation and deployment of a dedicated Covid-19 dashboard. The dashboard provides a comprehensive view of infections, hospitalisations, deaths, and vaccinations, giving a more detailed analysis of the impact of the virus to support decision-making and ensure people across the county receive the most effective and integrated care.
Now launched across NHS organisations in Kent and Medway, the health and social care professionals that use the KMCR have found it to make a positive impact on how they can treat patients, giving them more visibility of a patient’s conditions and enabling them to make faster and better informed clinical decision making.
This shared record is delivering a significant transformational change in Kent and Medway. As well as helping us improve the quality and safety of care for patients, it is underpinning our drive to integrate citizens’ records across health and social care.
Cantium’s implementation of Kent & Medway’s Shared Care Record is a crucial part of the digital transformation of both Kent’s health and social care and of the NHS.
The KMCR was commissioned on behalf of KCC, Medway Council, and Kent and Medway CCG, but it represents only the first step. Increasingly, NHS Trusts around the UK will need to evaluate how they harness their patient data into a single system, and aim to follow in the footsteps of KMCR by “delivering a significant transformational change” (Dr. Navin Kumta, Clinical Chair of NHS Kent and Medway CCG).
